Thromboprophylaxis in pregnancy and the postpartum period

The risk assessment scheme, weight-based dosing of low molecular weight heparin and determining duration.

Version 1.924 pagesUpdated 11 June 2024Hypertensive Disorders Working Group

Key recommendations

  • Risk assessment at the first visit, on admission and after delivery
  • LMWH dose according to body weight
  • Postpartum prophylaxis for 10 days to 6 weeks
  • Timing intervals around regional anaesthesia

Venous thromboembolism remains an avoidable cause of maternal death. Systematic risk assessment reduces the number of missed cases.

Note: A guideline is a recommendation and does not replace the clinician's professional judgement in an individual case. Departing from it is acceptable where the reasoning is documented.
